Catalog Description
The CHW Program trains and certifies students to work in the fields of public health, health care and social services. Community Health Workers (CHWs) provide a wide range of services, including health outreach and education, client-centered counseling and case management, and client and community-based advocacy. They work to reduce unequal rates of illness and death between different communities and to promote health equity or justice, prioritizing communities with the greatest health risks. Learning Outcomes 1. Identify basic health needs and evaluate health and human service resources within the community. 2. Demonstrate CHW professional skills and job readiness in the core competencies identified by the Community Health Care Worker Core Consensus Project (C3) and the needs of the local community.

To earn the Community Health Worker Certificate of Achievement, the requirements are to complete a minimum of 18 units with a passing grade of "C" or better in the following coursework: Core Courses (units) HS 10 Personal Health (3) HS 30 Introduction to Community Health Work (3) HS 31 Community Health Worker Skills (2) HSERV 50 Introduction to the Human Services (3) HSERV 52 Introduction to Counseling and Case Management (2) CWEE 99G General Work Experience Education (2) Approved Elective (3) Total Units: 18 No sequencing requirements.
